PAYMENT Payment must be made at the time of registration. NCMA accepts MasterCard, Visa, American Express, Discover, and personal/company checks. Purchase orders used for payment of an NCMA education training event must be accompanied by all appropriate supporting documentation. NCMA requires that you submit the purchase order number as well as the approval for training. All purchase orders submitted must include an individual registration form for each attendee (even if there are multiple attendees on one purchase order), the date and method by which payment will be guaranteed; and if different than registrant, a primary contact for the account. All registrations must be received prior to 5:00 CST on July 8, 2013. If not submitted by this deadline, you must register onsite. Each individual must complete a separate registration form.
FOREIGN PAYMENT Registration fees may be paid by credit card or by checks drawn on U.S. banks in U.S. ($) dollars. NCMA does not accept purchase orders from foreign governments and will not invoice them for payment.
CANCELLATION, SUBSTITUTION, AND REFUND REQUESTS All cancellations, substitutions, and requests for refunds must be completed in writing. Registrants who are unable to attend the event or need to request a substitution must e-mail their request to ncmawc2013@cteusa.com, on or before July 8, 2013. A $150 administrative fee will be applied to all cancellations received prior to July 8, 2013. Refunds will not be issued for cancellations received after July 8, 2013, 5:00 pm CST or for no-shows.
IDENTIFICATION BADGES Each attendee will be provided with an identification badge during on-site registration. This badge must be worn while attending all conference functions. Badge switching/sharing is prohibited. A valid government-issued ID, such as a driver’s license, is required at check-in.

* Nonmembers will receive a complimentary one-year NCMA membership with registration payment and event attendance. This complimentary membership will be effective August 2013–July 2014 and will be processed automatically within 10 business days following the event.
